This season, all Uniques can become or drop as Mythic Uniques. This simple change is what makes Pandemonium Fragments so coveted. In the Horadric Cube, you can transmute these fragments along with a Unique item to obtain a random brand-new Mythic Unique Item of the same class. While it is heavily reliant on RNG, it's a great way to turn unwanted Uniques into something potentially usable.

Slay the Corrupted Reaper
Pandemonium Fragments are dropped exclusively by the Corrupted Reaper, the brand-new lair boss. More specifically, these fragments are from opening the treasure hoard at the end of the boss fight. Opening the Lair Boss Hoard requires 1 Superior Lair Key. The Corrupted Reaper is a repeatable lair boss that can be farmed by most capable builds with relative ease. He is located in Zarbinzet, Hawezar, and can be summoned and challenged at any time.
Other sources of Pandemonium Fragments
According to the game, Pandemonium Fragments can be dropped from Glint of Hope Caches. However, based on community reports, the fragments do not drop from these caches. Aside from farming the Corrupted Reaper, there are currently no other known methods to farm these fragments.
