At long last, the fabled Secret Cow Level for Diablo 4 has been found. Fans of this franchise have been searching for this level as if it were the Holy Grail. The fascination with Secret Cow Levels started all the way back in Diablo 2 and persisted as a meme until it became reality. Finally, in 2026, the Secret Cow Level for Diablo 4 has been found, and here's how to get there.
Secret Cow Level Full Guide and Walkthrough
To get to the Secret Cow Level, players must first complete a series of hidden quests. This quest is rather tricky to complete because your journal won't track your progress. This guide starts off from the 3rd and final hidden cow level quest.
Secret Cow Level Hidden Quest Lord of Hatred
- Go to the Outcast’s Journal found at Mount Hefeatrus East Face. It should say “Blind are those that deny their own sins.”
- At Kyovashad Gatehouse, go to the Holy Cedar Tablets and choose option 3: Inscribe ‘Pride’ into the wood.
- Head back to the Outcast’s Journal, and the text should have changed to “...these machines make short work of even the vilest beast Hell can spawn, as though the Heavenly Father himself was hurling his mighty spears.”
- Head to areas on Skovos with the ‘Ballista’ enemy, like Observatory Approach. Destroy these enemies and other mobs in the area until an Amazonian Bolt is dropped.
- Take the bolt with you to Eastwood Bluff and into Idyllic Reach. Inside, you will find a Ballista where you can load and shoot the bolt, unlocking a new hidden area.
- Continue in the hidden area until you find a statue of Inarius, interact with it, and kill the enemy that spawns from it. Once slain, the demon will drop an item called Father’s Chains.
- Head to Antia’s Flame and interact with the Signal Brazier.
- Head back to Kyovashad Gatehouse, go to the Holy Cedar Tablets, and choose option 2: Inscribe ‘Anger’ into the wood.
- The Outcast’s Journal should now say “... the lifeless effigy stood uncowed, but once battered by anger’s relentless blows, it finally bowed its head, and the Amazon earned the first piece of her panoply.”
- Head to the Training Grounds and attack the Training Dummy until it drops an item called ‘Amazon Training Helm’.
- Equip the helm. Once equipped, you must defeat corrupted amazons found in Akarat’s Bulwark and equip each gear piece that drops until you have a full set of Amazon Training gear. (You must equip each piece that drops while doing the quest step, so lower the difficulty if you must.)
- Once all pieces have been found and equipped, explore Akarats Bulwark until you find an Elite Corrupted Amazon Captain. Defeating her will drop a Mother’s Rose.
- With the Mother’s Rose, head to the Temple of Courage and interact with the Signal Brazier. At this stage, the Amazon armor can be removed.
- Head back to Kyovashad Gatehouse, go to the Holy Cedar Tablets, and choose option 1: Inscribe ‘Fear’ into the wood.
- Go back to the Outcast’s Journal; it should now say “...on that auspicious day, I saw it sitting near the edge of life and death. Though I was trembling, it beckoned me to follow, and so I did.”
- For this step, dismiss your mercenary and head to the Golden Boughs and into the Ruins of Broken Reason. Head to the southwest edge of the ruins to find a Large Crow, interact with the crow, and follow it. Follow the crow until it goes back to where it started; it will drop an item called Hierophant’s Skull.
- Head to the Temple of Life and interact with the Signal Brazier. Once done, a special key called Hadopelagic Key should drop.
- Head to The Creaking Hulls in Hawezar and go to the end of the hook-shaped area. Stand on the wooden planks and Fish in the area, this should fish up a Locked Wooden Coffin.
- Unlock the coffin with the key and interact with it to enter Rathma’s temple. Head to the altar where Elias Mummified Finger can be found. Search and move around the immediate area until an item called Neyrelle’s Hand drops.
- Bring these items to the Horadric Cube: Stamina Potion, Rusted Bardiche, Neyrelle’s Hand. Transmute these items to receive the Trophy of the Faithful. Open the cache to receive ‘Torn Page.’
- Head to Zerae’s Shore, stay on the eastern edge to find a wooden rowboat called Outcast’s Boat.
How to Enter the Secret Cow Level
- At Kyovashad Gatehouse, go to the Holy Cedar Tablets and choose option 4: Inscribe ‘Greed’ into the wood.
- Go to the Outcasts Journal found at Mount Hefeatrus East Face. It should now say “...knowing it existed wasn't enough. I needed more. To see with my own eyes.”
- Head to the wooden rowboat on Zerae’s Shore in the Skovos Region and head to the Cow Island (Scylara).
- Go down into the Hecatombs and kill all the cows surrounding the circle.
Inside the Secret Cow Level
Once you step through the portal, you will be transported to an ornately decorated room. Outside of it are Tormented Bovine, cows that stand upright, wielding weapons. They don't drop anything of great significance. Explore the area until you find the Cow King.
The Cow King
The boss of this area is called The Cow King. He is nowhere near difficult, but he does drop a Mythic Unique called The Cow King's Crown.

Verdict
The item itself is clearly a joke, and maybe the real reward for completing the secret cow level was the journey and puns on the way. Getting to the Cow Level is a long and difficult process, but one that was definitely entertaining all the way through. While the Crown is useless, it is a good-looking transmog option for those who have it and can wear it proudly, knowing they have felled the Cow King.
